1. Where do I get a public health grant application?

2. When are applications accepted?

Application period opens typically August 1st and closes September 31st.   

3. Can I submit more than one application?

Yes, for example: a health clinic that provides medical services, dental services, and testing/counseling separately with different needs may apply per program.  However, applicants may identify one or more of the core public health service areas and may submit an single application that combines two or more health areas to be funded. 

4. Is a logic model required with all applications?

It is not required, for first time applicants.  Logic models are a useful tool for planning, evaluation and measuring projects success and are recommended.  Second year applicants should include logic models described in the program guidance document with their applications.   Example logic models for local projects will eventually be available on the Health First Hamilton County webpage for reference.   

5. Is funding for new projects only?

Building workforce on existing programs that increase access to care and linkages to care, reduce barriers to an identified community gap, and have measurable metrics of health improvement are applicable. For example, an existing program currently serving patients for chronic health issues like diabetes with ongoing care and education, and applicant is needing to add staffing, hours, days, or special event locations for additional program appointments with measurable improvement.  

Funding can be used to supplement increases in services but not to supplant existing funding sources that improved service.  

6. What is the award limit?

Currently there is not an established application limit on the request for funding. Eligible and complete applications will be evaluated in accordance with the review criteria stated in the guidance document and recommended funding made to the county commissioners for awarding. 

An application submission does not guarantee an award and in some cases, awards may be less that the amount requested.   

7. If awarded, how will the funds be given?

It is expected that funding will be disbursed in single payment after funding is received from the State of Indiana in January followed by the local claims processing procedures in the form of a MOU/Contract per Indiana Code.
